Assist Life Stage, Transition

Our Assist Life Stage, Transition services help participants navigate important milestones, such as transitioning from school to further education or employment or moving to a new living arrangement.

Assist - Personal Activities

Supporting you or your loved one with personal care, such as bathing, showering, applying creams, assisting with dressing, toileting, and continence management while maintaining privacy. All in the comfort of your own home.

Travel/ Transport

Transport services include shopping, medical appointments, social activities, and other community access trips. We provide safe, reliable, secure transport assistance to ensure a comfortable journey for you and your carers.

We understand that the level of transport assistance requ ired for each person is unique, and we work closely with you to ensure your individual needs are met.

Community Participation

Supports to help or supervise you to take part in community, social, recreational, or economic activities. Through community par ticipation capacity building, we aim to empower individuals with disabilities so they can have an active role within their community, and develop a sense of social inclusion and belonging, which can have a positive impact on their overall well – being.

Assistance with Daily Life

Supports to assist you with your personal tasks during day – to – day life so you can live as independently as possible. These supports can be provided individually in a range of environments, including your own home. Everyday househ old tasks are made easy with a clean and tidy home that you can be proud of.

Household cleaning services, meal preparation and grocery shopping are just a few of the services we provide to help you maintain a healthy, safe and independent lifestyle.

Modation / Tenancy

Supports to help you find individual and innovative living and housing options. We are able to find a suited home to your needs. We assist you in searching, meeting, and matching with hosts and co – residents (if applicable), by fixing a m eeting and ensuring that the funding and other arrangements are made well before time.

Assist Access / Maintain Employ

Supports to help your employment requirements. We support people with disabilities when they need a little extra help to find a job, ret ain a job, or exit from a job. We also ensure that the entire job seeking process, and the recruitment process are stress – free for our participants, and we assist them throughout, helping them in whatever way they seem necessary.

Household Tasks

We provide support with domestic activities such as cleaning, tidying, laundry, and general household maintenance. We support participants in managing their homes, promoting independence, and ensuring a comfortable living environment.

Behaviour Support Implemention

We actively work to put into practice the personalised strategies developed in your behaviour support plan. Our role is to ensure these strategies are effectively integrated website info.docx into your daily life, providing you with the tools and support needed for pos itive behaviour.
